First of all go there:

[glow=black,1,500]D:\Riot Games\League of Legends\RADS\projects\lol_air_client\releases\0.0.0.82\deploy\assets\images\buddyIcons[/glow]

or where your league of legends are installed.

2: Open Photoshop or Gimp or any other image editing software.

3: Create 128x128 document put an image u like on it and save it as .jpg at max quality with name profileIcon28 or profileIcon27 or 26 till 0 just change the last number

then put your image in there [glow=black,1,500]D:\Riot Games\League of Legends\RADS\projects\lol_air_client\releases\0.0.0.82\deploy\assets\images\buddyIcons[/glow].

you have to use skin installer to add new skins ;P
