I think real icons were introduced with T1 (Kamael). A red rectangular icon at the title is the way l2 client used to show enemies back in the day.

 

You can trust me, I tested all relations in Prelude Beta, C1 and then C4.

I am sure my friend . I remember in C1, because it interests me, an icon with punch . 
 
I remember wrong ? It was not on c1?

Definitely not on C1.

 

A red rectangle indicates an enemy. C1 Has that red rectangle on:

Pledge war

Alliance war

For pledge/ally leader, the icon will include crossed swords (default C1 PTS clan war implementation will obscure this icon during clan war and only show if there is only an alliance war or you have personally surrendered to an associated clan war)

 

 

During siege, an icon may be drawn in the center of the red rectangle:

Flag (attacker leader)

Crown (defender leader)

Sword (any attacker)

Shield (any defender)

P.S. A punch icon may have already become available in C4 (C3 pledge changes). T1 added those large square icons above the title, I thought you were referring to them at first.
