I used this photo :

For the start we can use the healing tool for the imperfections..

Now type Ctrl+j to make a copy of the background, choose your colors foregorund-black and background-white and then go to Image->Adjustments->Gradient Map and click ok.

With the eraser tool erase all but the girl. Like this:

Click on the first layer and make a selection of the eyes and then make a copy via layer.

Drag the eyes layer on top of all :

 

54.jpg?w=470&h=425

 

Use the dodge tool on the iris of the eye:

 

64.jpg?w=470&h=406

 

Make a new layer and with the black brush tool make something like this :

 

75.jpg?w=470&h=399

 

Now use Gaussian Blur with radius 2.0

I advise you to make a new layer for every peace of brush you use and then change the layer to multiply.

well one of your parts is missing...
