Yes it is possible to get it from .png file as the file name can be altered. However, it would be bigger in size then an avarage png file which should get your attention, and you shouldn't open it. Also when you open a rat file, it saves in your registry folders hidden, and the file doesn't open. If you ever open a file which loads and don't open it most likely is a rat. Before opening, extracting any files from sites like these, upload to virus scan online or for more safety just use sandbox
