Well, first of all: I suggest you register and login in that website. Then, in (1) Choose any amount of money in USD: - custom amount and add ~5$ or so (or 4.90$). In (2) Choose how you would like to divide up your contribution: you can do what you like! Then, enter you email address and, if you want, you can choose to get it as a gift code (link). Or just pay the money - that's it! You'll get the link in your E-Mail. The rest should also be very obvious.
