If i have malaria lvl 4 and not kill the mob, and later came back for more malaria the mob give malaria lvl 4 or it start with 1?

If you already have malaria 4, and go back for malaria again while you still have malaria your malaria will simply go to level 5 and decrease your c.speed.

 

However if you dont have malaria [if its over] you will simply start from 1.
