We Will turn this image:

to something like this:

Open the image in photoshop and paste a paint peel texture over it. I used this texture:

Now select the paint peel texture layer and change the blending mode to "Luminosity" and turn the opacity down to 32%.

Adjust the paint texture so it will fit around her eyes and look like this:

Now we will create a mask in the paint peel texture layer and remove the cracks from her eyes and other unwanted areas. Add layer mask like this:

Next, select Paint Brush tool and set the color to black and paint through her eyes, eye brows and the rest of area around her face. Your end result should be something like this

As some area in the forehead in empty so we will add another cracks texture to fill it up. I used this image:  http://www.atpm.com/8.04/travels/images/MudCracks.jpg

Change the blending mode of new texture layer to "Luminosity" and turn the opacity down to 32%.

Adjust the new texture so it will fit around the empty area on her forehead.

Add layer mask.

Select Paint Brush tool and set the color to black and paint through all over the image expect for the empty area on her fore head.

It should look something like this:

Through the Layers palette add a Channel Mixer and set these values for the channel mixer.

Now we will change the color of her eyes. So, take a pen tool and draw around the eye ball as shown.

With the pen tool still selected, right click on the image and select Make Selection and hit OK.

Select the original picture layer. Press Ctrl+U on on your keyboard to open up Hue/Saturation window and enter these value.

Repeat the same process for second eye so in the end you will get a result like this:

Play with the setting of channel mixer to get more nice results like this one:
